The following items must be completed before your child will be considered for enrollment:

  • Parent/Guardian tour of the AppleTree facility
  • Parents/Guardians have met their child’s teacher, the AppleTree Director, and the Financial Director or Asst. Financial Director
  • Immunization and shot records have been submitted.
  • $35 registration fee and the first month’s tuition have been paid
  • Income eligibility form has been completed and submitted
  • Parent Acknowledgement Form has been completed and submitted
  • Current Immunization Records
  • Income Eligibility Application (August and January)
  • Signed Handbook Agreement
  • Current Picture
  • Financial Contract
  • Signed Orientation Attendance Form

The following items must be returned to Manzano Christian School office before your child will be considered for enrollment:

  • $50 Registration Fee (non-refundable)
  • Enrollment Application (Notary)
  • Medical and Emergency Information and Authorization Form (Notary)
  • Copy of child’s Birth Certificate
  • Copy of Social Security Card
  • Copy of Insurance Card
  • Current Immunization Records
  • Income Eligibility Application (August and January)
  • Record Transfer Request (if applicable)
  • Signed Handbook Agreement
  • Current Picture
  • Financial Contract
  • Signed Orientation Attendance Form
