The following items must be completed before your child will be considered for enrollment:
- Parent/Guardian tour of the AppleTree facility
- Parents/Guardians have met their child’s teacher, the AppleTree Director, and the Financial Director or Asst. Financial Director
- Immunization and shot records have been submitted.
- $35 registration fee and the first month’s tuition have been paid
- Income eligibility form has been completed and submitted
- Parent Acknowledgement Form has been completed and submitted
- Current Immunization Records
- Income Eligibility Application (August and January)
- Signed Handbook Agreement
- Current Picture
- Financial Contract
- Signed Orientation Attendance Form
The following items must be returned to Manzano Christian School office before your child will be considered for enrollment:
- $50 Registration Fee (non-refundable)
- Enrollment Application (Notary)
- Medical and Emergency Information and Authorization Form (Notary)
- Copy of child’s Birth Certificate
- Copy of Social Security Card
- Copy of Insurance Card
- Current Immunization Records
- Income Eligibility Application (August and January)
- Record Transfer Request (if applicable)
- Signed Handbook Agreement
- Current Picture
- Financial Contract
- Signed Orientation Attendance Form
